This form is to request an RF Tag for a new vehicle or to replace an existing RF Tag. One form is needed for each vehicle request. The address on the vehicle registration must match the address within Lido Isles. Thank you.

Please list ALL RF Tags that are registered to your home with the corresponding tag number. These tag numbers can be found on your current RF Tag on your windshield. ALL RF TAGS NOT LISTED ON THIS FORM WILL BE DELETED FROM THE SYSTEM. If you are a new homeowner, write NONE in this section.

Please upload all vehicle registrations into one file for all new or replacement tags. Your address must be listed on the registration. For new homeowners/renters, you will have 30 days to submit an updated registration with your new current address.
